Black Belts and Master Instructors Workshop

In consideration for the attendance and participation in the martial arts training offered by Lincoln Family Martial Arts Academy, Inc., I, the undersigned participant (or parent/legal guardian of the participant if under 18 years of age), acknowledge the existence of certain inherent risks in this type of training and hereby agree to assume all such risks. I further release and hold harmless Lincoln Family Martial Arts Academy, its management, assigned staff, fellow students, and/or agents from any liability resulting from loss, including but not limited to personal belongings or bodily injury, occurring on premises leased or owned by Lincoln Family Martial Arts Academy. I also affirm that I (or the participant) am physically fit to take the prescribed course of instruction and do so voluntarily and of my own free will in exchange for an agreed-upon fee. I understand that there are no refunds under any conditions. I also give permission to be added to the email list.
